Online Near Miss and Hazard Observation Reporting

In order to make it easier for our workforce to engage with the reporting of near misses and hazard observations, as well as recording positive/negative health, safety and environmental incidents on site, we have developed a simple to use questionnaire using Microsoft Forms.

The advantage of using the Microsoft platform means anyone with access to a smart phone can use the system – it is not just limited to people with access to company IT equipment. By saving a bookmark to a phone’s home screen, the questionnaire can be used like a conventional app.

Posters are displayed around site with a copy of the unique QR code which takes you to the form to be completed. The posters also have guidance on how to save a bookmark to your phone’s home screen. People can choose to remain anonymous when filling in the questionnaire if they wish to, and the questionnaire only takes a couple of minutes to complete.

The Microsoft Forms system automatically generates tables and graphs, giving instant statistical analysis. The platform can also be set up to automatically email site staff when a report comes in.

Since introducing the online system, we have seen an increase in reporting from all members of our site team.
