All accidents, incidents and near misses should be recorded and used for lessons learnt, to ensure continuous safety improvement on site.

A great deal can be learnt from trends in accident reporting.

The reporting process should capture all relevant information, but also be quick and simple to complete.

The Considerate Constructors Scheme have put together an incident report template for smaller sites to use.

This enables operatives to complete the form, immediately after becoming aware of an incident.
